Certified Copies of the Death Certificate
Certificates are obtained for you from the State. The cost is $13 for the first copy and $6 for each additional copy. Death Certificates are needed for the some of the following purposes.
*When the deceased is the insured. *When the deceased is listed as a beneficiary of a another personís policy.

*Some credit cards have credit life insurance.
*Contact employer for information
*Cars, Trucks, Trailers, Motorcycles, Boats, etc.
Real Estate Titles
*Contact your attorney or a realtor for help.
Stocks, Bonds, and other Securities
*You may need to consult a broker.
Income Tax Returns *Internal Revenue Service requires a certified copy if a refund is expected with the next filing.
*In some cases such as applying for a Lump Sum Death Benefit, Surviving Spouse Benefit or Dependents Benefits.
*Checking Accounts that only the deceased signs on.
*Certificates of Deposit
*Automatic Withdrawal Payments.
